The Chaplain provides spiritual care and support to patients and families, honoring their individual beliefs and preferences. This role is an integral part of the interdisciplinary team, ensuring that spiritual needs are thoughtfully addressed throughout the hospice journey.

Schedule: This is a full-time, 30-hour per week position with flexibility in scheduling. The Chaplain may work either three 10-hour days per week or four 7.5-hour days per week, depending on preference. The role requires participation in interdisciplinary team (IDT) meetings every other Thursday and every other Friday at 7:00 AM.

There is an on-call rotation of approximately one full weekend every 6–7 weeks. Weekend call hours include Friday from 5:00 PM to 7:00 PM, Saturday from 9:00 AM to 5:00 PM, and Sunday from 10:00 AM to 4:00 PM. While on-call, the Chaplain is responsible for covering Unity Hospice’s full service area. In addition, this role includes a holiday rotation of approximately one holiday every 1.5 years.

Territory: Based in Sturgeon Bay and surrounding area. Covering Northern Door County and Washington Island. Travel throughout the territory is required.

Key Responsibilities:

  • Provide spiritual counseling and support to patients and families as requested
  • Complete spiritual assessments and develop individualized spiritual care plans
  • Coordinate with patients’ clergy and facilitate spiritual services, prayer, or sacraments
  • Assist families with funeral planning when requested
  • Collaborate with Volunteer Services to connect spiritual care volunteers with patients
  • Serve as a liaison to the local religious community and provide education on hospice care
  • Partner with the bereavement team on memorial services and programs
  • Participate in interdisciplinary team (IDT) meetings and contribute to care planning
  • Support process improvement initiatives related to spiritual care services

Requirements:
  • Bachelor's degree from an accredited school of theology, religious studies, or related studies and/or certification in hospital or pastoral ministry required. Master's degree in theology, divinity, or pastoral studies preferred.
  • Board Certification by the NACC (National Association of Catholic Chaplains or APC (Association of Professional Chaplains) or working toward certification preferred.
  • Two units of Clinical Pastoral Education are preferred.
  • Experience in hospice and palliative care and an understanding of hospice and palliative philosophy is preferred.
  • Valid Wisconsin Driver’s License, reliable transportation and proof of current automobile insurance coverage
