Life Care Centers is seeking a compassionate, licensed Physical Therapist Assistant to join our Healthcare & Medical Services team, supporting high-quality rehabilitation and senior care in a warm, family-focused environment. In this role, you will work under the direction and supervision of a licensed Physical Therapist to deliver hands-on treatment to residents and patients in our facility, helping them regain strength, mobility, and independence following illness, surgery, or injury. You will implement individualized treatment plans, lead therapeutic exercises, assist with gait and balance training, and support functional mobility activities such as transfers and activities of daily living, always prioritizing safety, dignity, and patient comfort. As a key member of a collaborative, interdisciplinary care team, you will communicate regularly with physical therapists, nursing staff, and other healthcare professionals to monitor progress, adjust interventions within your scope of practice, and contribute to positive clinical outcomes. You will document all treatments and patient responses accurately and timely in the electronic medical record, adhering to regulatory standards, payer requirements, and Life Care Centers’ policies. Your day-to-day work will include educating patients and families on home exercise programs and safe mobility strategies, reinforcing the therapist’s instructions in clear, compassionate language that supports understanding and long-term success. You will consistently follow infection control procedures and safe patient-handling practices, demonstrating sound clinical judgment and professional ethics in every interaction.
