CarePartners of Connecticut is seeking a Nurse Extern who is passionate about community health, collaboration, and patient-centered care. As a Nurse Extern, you will work under the direct supervision of registered nurses and care managers to support coordinated, high-quality care for our members across the CarePartners network of partnered providers. This role is designed for nursing students who want to strengthen their clinical skills while learning how integrated health coverage and provider collaboration can improve health outcomes. In this position, you will assist with basic patient assessments, including obtaining and documenting vital signs, gathering health histories as appropriate, and accurately entering information into electronic health records. Working closely with the healthcare team, you will support care coordination activities such as follow-up calls, appointment and test follow-through, and reinforcing patient care plans developed by licensed clinicians. You will help bridge communication among patients, families, providers, and internal teams to ensure members understand their care instructions and have access to the services they need. You will be expected to adhere to established clinical protocols, infection control procedures, and safety and privacy regulations at all times, demonstrating professionalism, compassion, and respect in every interaction. Under supervision, you may assist with basic clinical tasks that support nursing care, observe and promptly report changes in patient status, and document findings in a clear, timely, and accurate manner. You will also help identify barriers to care, such as transportation or medication access issues, and escalate them to the care team so that appropriate solutions can be coordinated. Because CarePartners is committed to improving community health, the Nurse Extern will also be involved in outreach and education efforts focused on prevention, wellness, and chronic disease management. You will collaborate with an interdisciplinary team to support initiatives that enhance patient experience, promote quality and safety, and strengthen the effectiveness of our provider network.
