Tufts Medical Center is seeking a Weekend Clinical Manager – Home Health & Hospice (Sign-on bonus eligible) to oversee and coordinate clinical operations for our home-based and hospice services during weekend hours. As a key clinical leader within our Healthcare & Medical Services department, you will ensure that patients and families receive safe, compassionate, and evidence-based care in their homes and in hospice settings, while supporting and guiding a multidisciplinary field team. In this role, you will provide real-time clinical leadership for nurses, therapists, social workers, and hospice staff across our home health and hospice programs on weekends. You will oversee triage of new and existing patients, support complex clinical decision-making, and ensure that care plans are implemented appropriately and updated based on changing patient needs. You will review and authorize clinical interventions, monitor symptom and pain management for hospice patients, and help staff navigate acute changes in patient status, including escalation to on-call physicians or higher levels of care when needed. You will be responsible for coordinating weekend staffing and visit assignments, balancing patient acuity, staff competencies, and geographical considerations. Using the electronic health record and established workflows, you will monitor visit completion, documentation quality, and timely communication among the care team. You will help ensure that all services delivered over the weekend comply with regulatory and accreditation requirements, including CMS and Joint Commission standards, as well as internal Tufts Medical Center policies. A key aspect of this position is coaching and supporting clinical staff in the field. You will be available as a resource for complex clinical questions, provide feedback on documentation and care planning, and promote best practices in home health and hospice care. You will participate in case reviews, help address safety and infection control concerns in the home setting, and collaborate with leadership to identify trends, address performance issues, and contribute to ongoing quality improvement initiatives.
